If you are a current Reliance participant, you can now access our laboratory reports in the Reliance Community Health Record portal.

If you are not a current participant, access to the Reliance Community Health Record is available at no cost. Please email info@RelianceHIE.org or call (855) 290-5443 to learn more about enrolling.
